מדליה מצופת אמייל. פנים: מגן דוד שבמרכזו הכיתוב "לזכר הקונגרס הראשון תרנ"ז-תרע"ז", ובשש פינותיו סמלים שונים: שבעה כוכבים, אלומת שיבולים, ועוד. מסביב הכיתוב: "אם אין אני לי מי לי?". גב: חריטה, עם עיטורי עלים ופרח: "יעקב שורץ, ורשה תרע"ז".
