דף כפול, עם שני קמעות מודפסים, "שמירה" ליולדת זכר, ו"שמירה" ליולדת נקבה. וילנא, דפוס R. M. Romm, [תר"ל] 1870.
בראש הקמעות, מופיעות כותרות: "מזל טוב לזכר" ו"מזל טוב לנקבה". תוכן הקמיעות דומה. בראש מופיע מזמור תהילים כ"א ("שיר למעלות אשא עיני"), ולאחריו שמות האבות ופסוקים נוספים, כשבמרכזם מסגרת מעוטרת עם השמות "אדם וחוה; פנימה שד'י; קרע שטן".
